Testosterone Replacement Therapy: Is It Right for You?
Many aging men with low testosterone report improved energy levels, sex drive, and mood after testosterone treatment. If testosterone is low, why not replace it?
Not so fast. A low testosterone level by itself doesn’t need treatment. Testosterone replacement therapy can have side effects, and the long-term risks and benefits aren’t known. Only men with symptoms of low testosterone should consider testosterone replacement. Talking with your doctor
Low Testosterone: Subtle Symptoms
The symptoms of low testosterone are sometimes obvious, but can be subtle. Testosterone levels decline naturally in men as they age over decades, and symptoms may appear slowly. Low testosterone symptoms include:
- Low sex drive (libido)
- Erectile dysfunction
- Fatigue and poor energy level
- Difficulty concentrating
- Depression
- Irritability
- Low sense of well-being
If a man has symptoms of low testosterone and tests show he has a low testosterone level, a doctor may suggest treatment. For millions of men who have low testosterone levels but no symptoms, no treatment is currently recommended.
Forms of Testosterone Supplements
Testosterone replacement therapy is available in several forms. All can improve testosterone levels:
- Skin patch (transdermal): Androderm is a skin patch worn on the arm or upper body. It’s applied once a day.
- Gels: AndroGel and Testim come in packets of clear testosterone gel. Testosterone is absorbed directly through the skin when you apply the gel once a day. AndroGel also comes in a pump that delivers the amount of testosterone prescribed by your doctor.
- Mouth patch: Striant is a tablet that sticks to the upper gums above the incisor, the tooth just to the right or left of the two front teeth. Applied twice a day, it continuously releases testosterone into the blood through the oral tissues.
- Injections and implants: Testosterone can also be injected directly into the muscles, or implanted as pellets in the soft tissues. Your body slowly absorbs the testosterone into the bloodstream.
Why not a simple testosterone pill? Oral testosterone is available. However, some experts believe oral testosterone can have negative effects on the liver. Using other methods, such as skin patches, gels, orally disintegrating tablets, or injections, bypasses the liver and gets testosterone into the blood directly.
Benefits of Testosterone Therapy
What can you expect from testosterone treatment? It’s impossible to predict, because every man is different. Many men report improvement in energy level, sex drive, and quality of erections. Testosterone also increases bone density, muscle mass, and helps improve obesity and insulin sensitivity in some men.
Men also often report an improvement in mood from testosterone replacement. Whether these effects are barely noticeable, or a major boost, is highly individualized.
Karen Herbst, MD, PhD, an endocrinologist at University of California-San Diego, specializes in testosterone deficiency. She estimates about one in 10 men are “ecstatic” about their response to testosterone therapy, while about the same number “don’t notice much.” The majority have generally positive, but varying responses to testosterone replacement.
Risks of Testosterone Therapy
The common preparations of testosterone replacement have frequent, mild side effects. Testosterone side effects most often include rash, itching, or irritation at the site where the testosterone is applied.
Testosterone replacement so far seems to be generally safe. Experts emphasize that the benefits and risks of testosterone therapy are unknown, because large clinical trials haven’t yet been done.
There are a few health conditions that experts believe testosterone therapy can worsen:
- Benign prostatic hypertrophy (BPH): The prostate grows naturally under the stimulation of testosterone. For many men, their prostates grow larger as they age, squeezing the tube carrying urine (urethra). The result is difficulty urinating. This condition, benign prostatic hypertrophy, can be made worse by testosterone therapy.
- Prostate cancer: Testosterone can stimulate prostate cancer to grow. Most experts recommend screening for prostate cancer before starting testosterone replacement. Men with prostate cancer or elevated prostate specific antigen (PSA) should probably avoid testosterone treatment.
- Sleep apnea: This condition can be worsened by testosterone replacement. It may be difficult for a man to detect this himself, but his sleeping partner can often tell. A sleep study (polysomnography) may be needed to make the diagnosis.
- Erythrocytosis (high blood counts): Testosterone supplements increase the red blood cell count—possibly making blood “thicker” and perhaps more prone to clotting. Men with the highest blood cell counts may be at greater risk for health problems with testosterone therapy. Simple blood tests can track your red blood cell count.
- Congestive heart failure: Men with severe congestive heart failure should usually not take testosterone replacement, as it can worsen the condition.
It will be years before large clinical trials bring any answers on the long-term benefits and risks of testosterone therapy. As with any medicine, the decision on whether the possible benefits outweigh any risks is up to you and your doctor.
Testosterone Replacement vs. Performance-Enhancing Steroids
Isn’t taking testosterone replacement basically the same as taking steroids, like so many disgraced pro athletes? It’s true that anabolic steroids used by some bodybuilders and athletes contain testosterone or chemicals that act like testosterone.
The difference is that doses used in testosterone replacement only achieve physiologic (natural) levels of hormone in the blood. The testosterone forms some athletes use illegally are in much higher doses, and often combined (“stacked”) with other substances that boost the overall muscle-building (anabolic) effect.

1. Set the Table
Children in families with more structured mealtimes exhibit healthier eating habits. Among middle- and high-school girls, those whose families ate together only once or twice per week were more than twice as likely to exhibit weight control issues, compared with those who ate together three or four times per week.
Of course, the notion of a 6 p.m. dinnertime and then everyone into their pj’s is a quaint one, but it’s hardly realistic in a society where our kids have such highly scheduled social lives that the delineation between “parent” and “chauffeur” is sometimes difficult to parse. While we can’t always bring the family together like Ozzie Nelson’s (or, heck, even like Ozzy Osbourne’s), we can make some positive steps in that direction. One busy family I know keeps Sunday night dinner sacred—no social plans, no school projects, no extra work brought home from the office. Even keeping the family ritual just once a week gives parents the opportunity to point out what is and isn’t healthy at the dinner table.
2. Eat More Whole Foods and Fewer Science Experiments
Here’s a rule of healthy eating that will serve you well when picking out foods for your family: The shorter the ingredients list, the healthier the food. (One of the worst foods we’ve ever found, the Baskin-Robbins Heath Shake, has 73 ingredients—and, by the way, a whopping 2,310 calories and more than 3 days’ worth of saturated fat! What happened to the idea that a milk shake was, um, milk and ice cream? Let’s be grateful that Baskin-Robbins finally pulled this monstrosity from their menus.) The FDA maintains a list of more than 3,000 ingredients that are considered safe to eat, but we’ve found reasons for concern for a number of the additives on that long list, and any one of them could wind up in your next box of mac ’n’ cheese.
According to USDA reports, most of the sodium in the American diet comes from packaged and processed foods. Naturally occurring salt accounts for only 12 percent of total intake, while 77 percent is added by food manufacturers.
3. Drink Responsibly
Too many of us keep in mind the adage “watch what you eat,” and we forget another serious threat to our health: We don’t watch what we drink. In fact, according to research from the University of North Carolina, Americans now slurp up nearly 25 percent of their calories in liquid form—nearly double the rate we used to drink just 20 years ago. One study found that sweetened beverages constituted more than half (51 percent) of all beverages consumed by fourth- through sixth-grade students. The students who consumed the most sweetened beverages took in approximately 330 extra calories per day, and on average they ate less than half the amount of real fruit than did their peers who drank unsweetened or lightly sweetened beverages.
One important strategy is to keep cold, filtered water in a pitcher in the fridge. You might even want to keep some cut-up limes, oranges, or lemons nearby for kids to flavor their own water with. A UK study showed that in classrooms with limited access to water, only 29 percent of students met their daily needs; free access to water led to higher intake.
Another important strategy: Be extra careful about the juice you purchase. Too many “juices” are little more than sugar water masquerading as the real thing. Ocean Spray Cran-Raspberry, for instance, has just 15 percent real fruit juice. The other 85 percent? High-fructose corn syrup and water. Make sure the juice you buy says “100 percent Fruit Juice” on the label, and try to choose one made from a single fruit, not a mix of high-sugar fruits like white grapes, which are commonly used in fruit juice blends.
4. Beware of Portion Distortion
Snack portions aren’t the only things that have increased wildly in size. Since 1977, hamburgers have increased by 97 calories, French fries by 68 calories, and Mexican foods by 133 calories, according to analysis of the Nationwide Food Consumption Survey.
A study published in the American Journal of Preventive Medicine looked at 63,380 individuals’ drinking habits over a span of 19 years. The results show that for children ages 2 to 18, portions of sweetened beverages increased from 13.1 ounces in 1977 to 18.9 ounces in 1996.
One easy way to short-circuit this growing trend? Buy smaller bowls and cups. A recent study at the Children’s Nutrition Research Center in Houston, Texas, shows that 5- and 6-year-old children will consume a third more calories when presented with a larger portion. The findings are based on a sample of 53 children who were served either 1- or 2-cup portions of macaroni and cheese.
5. Snack With Purpose
There’s a big difference between mindless munching and strategic snacking. Snacking with purpose means reinforcing good habits, keeping your metabolic rate high, and filling the gaps between meals with the nutrients your child’s body craves.
Chew on this piece of trivia: In the 20 years leading up to the 21st century (1977 to 1996), salty snack portions increased by 93 calories, and soft drink portions increased by 49 calories.
Combat portion distortion by eating healthy snacks: Triscuits and peanut butter; string cheese; a sandwich bag filled with homemade popcorn; or that classic of kid’s snacktime nourishment, ants on a log.
6. Never Skip Breakfast
Yes, mornings are crazy. But they’re also our best hope at regaining our nutritional sanity. A 2005 study synthesized the results of 47 other studies that examined the impact of starting the day with a healthy breakfast. Here’s what they found:
People who skip breakfast are more likely to take up smoking or drinking, less likely to exercise, and more likely to follow fad diets or express concerns about body weight. Common reasons cited for skipping were lack of time, lack of hunger, or dieting.
Bad news. Sure, it would seem to make sense that skipping breakfast means eating fewer calories, which means weighing less. But it doesn’t work that way. Consider:
People who eat breakfast tend to have higher total calorie intakes throughout the day, but they also get significantly more fiber, calcium, and other micronutrients than skippers do. Breakfast eaters also tended to consume less soda and French fries and more fruits, vegetables, and milk.
Breakfast eaters were approximately 30 percent less likely to be overweight or obese. (Think about that—people who eat breakfast eat more food, but weigh less!)

Are athletes different? If you’re active enough in your sport to be called an athlete, or if you compete on any level at all, do you need to eat differently?
The answer: you may not need different nutrition, but you do have to give your body what it needs. That may require something different from the regular diet of a non-active person. Any athlete, whether Masters, league or elite, must regard their body as a payoff machine. If you have a good amount of fiber in your system, but not enough protein to get through a game – you won’t have enough physical energy to compete for very long, though you’ll be just great in the bathroom. Or, let’s say you’re more athletic than an actual athlete, and you plan on a long and leisurely hike on the following morning – do you eat carbs or protein the night before? Should you take supplements before or after a workout or training session?
An athlete’s diet should be specific to the way the body is going to be used. There should also be some attention paid to timing. For example, if you’re running a long race, like a marathon, you want to spend the week before eating a lot of carbs in order to train your muscles to store carbohydrates better. But they have to be the right kind of carbs; the long storage, slow burning ones. A candy bar will give you a big burst of energy, then let you crash because you’ve burned up all that carb energy fast. A pasta dinner the night before the race will provide slow burning carbs to be used for energy while you run mile after mile.
But if you do a power sport, such as a team game with lots of running and jumping, or sprints on your feet or a bike, you need more protein. Protein provides the muscles with the nutrients they need to keep working hard and fast. There are a number of ways to get the needed protein to your tissues; the main two methods are eating animals, and taking supplements. There are benefits to both. If you eat high quality creature protein – chicken breasts, tuna fish, turkey legs – your body will absorb these natural protein sources easily. If, on the other hand, you get your protein from supplements, it may not always be in a form your body can fully use. Some imported nutrients may be full of impurities or not high enough in quality to be absorbed by the body. You may have to take different brands of supplements until you find the one that works for you. But to have your body obey your desires in a competition, training session or conditioning workout, it’s best to combine both natural and bottled proteins.
Suppose you know that creatine will allow your body to work harder for a longer period of time, so you want to feed it to your muscles. However, a pound of steak contains only about two grams of creatine, and you want to more of this protein to be absorbed by, and stored in, your muscles for a game the following day. The best way to get it into your muscles is with a combination of a natural source (animal products), amino acids (essentially predigested proteins that will make creatine) and a supplement of creatine itself. With the combination, your muscles will absorb more creatine than they would with any one element alone.
This is why experts know that nutritionally, vitamins and minerals are better absorbed when they are taken with natural sources of the supplement. Eat some fruit and take some vitamin C at the same time, and more of the pill form of the vitamin will be absorbed.
You can learn a lot of nutritional information from other athletes or from studying nutrition. But the most important thing to remember is that everyone’s body is different, includign yours. You may load up perfectly on carbs and still bonk at mile 19. Or you can stuff yourself with protein and still get tired during a hard hitting game. Maybe you could last long enough to win with a poor diet when you were younger, but your protein needs changed over the years. You think your athletic ability has deteriorated – when in fact, all you need to do is to give your body what it now needs. That’s why it’s a good idea to do what most Olympic level athletes do: keep a food, training and competition diary. If you ate a lot of roma tomatoes for dinner, then did good the next day, write down how you do the next time you eat roma tomatoes. If your diary shows that those tomatoes have a good effect on your athletic ability, you’ll know more about how to feed your body.
Pay attention to your diet, to what you eat and when, and your results will improve more than you ever thought possible.

You may already know the importance of natural antioxidants inside the membranes of your cells (vitamin E is the most common in the skin): protecting you against free oxygen radicals in the membrane and lipid portions of the cell. They’re especially important for protecting your skin, because they help thicken your epidermis while the sun quickly depletes levels of vitamin E. Your body will replenish its own vitamin E if you are eating smart, but adding some extra vitamin C (which protects the water soluble portions of your cells can help decrease the appearance of wrinkles and improve the formation of collagen and elastin. Only certain types of vitamin C will penetrate the skin – one called L-ascorbic acid does this particularly well. To work, it must be in a concentration of at least 10 percent and must be kept acidic. So, you can’t just rub oranges on your face and expect it to work. L-ascorbic acid gets oxidized by the sun, rendering it ineffective, so use it at night. Niacin (vitamin B3) and panthothenic acid (vitamin B5) are other vitamins (taken orally or topically) that are good for the skin. In fact, topical niacin helps prevent injury caused by the sun, and increases the level of certain fats and protein in the skin, which improves its barrier function, and it helps reduce the yellowing of skin that’s associated with glycation (the yellowing can disappear between four and 12 weeks of use).

Many women spice things up with far-fetched fantasies; they’re exciting and not relationship-threatening. So go with it. During sex, whisper crazy fantasies (that involve her!). Tell her where in public you’d like to go down on her, say. Or give her those two men in bed—by being both of them, Paget says. “Be the wild guy, then switch and be the sensitive lover who lies on top and kisses gently.”

But she’s often too shy to act. More than a third of the women we surveyed said that during sex they’re thinking of dirty things they’re embarrassed to talk about. One in four is imagining a position she’s afraid to ask you to try.
“Often her hesitancy to speak up is related to other inhibitions, like worrying about how she looks or tastes,” says Yvonne K. Fulbright, Ph.D., the author of Sultry Sex Talk to Seduce Any Lover. So to start, describe senses: how great she smells, how good she looks or feels. If she says something, respond. If she hints at something, go for it. The more you show your excitement—and the more you lead—the more comfortable she’ll be.

It’s a cliché, but a true one. More than one in four women said they thought about household chores during sex, and 20 percent thought about work. “It’s just the way my mind operates,” says Stephanie, 27. So bring her back. Our poll’s top suggestion: Turn up the passion—and let her feel it. “When she’s bored, it’s because the sex is routine. So take charge. Move her around and be more assertive,” says Davidson. Hold her tighter, run your nails up her legs, pin her arms down (if she’s comfortable with that). A lot of little things can make her take notice. What laundry?

Yes, she thinks about other men during sex. Sometimes she even fantasizes about them. Ouch! But she’s conflicted. Consider: 51 percent of respondents said it was natural to fantasize about other men during sex, but 74 percent said they’d be hurt if you fantasized about other women.
Focus her attention. Ask her, “What do you want me to beg for?” Fulbright says. “It gives her the sense that she’s steering the action.” And she’ll know you’re the one giving her what she wants.
